Fill out the registration form
1.
At Mary Immaculate, to begin Baptism preparation, we want to get to know you more! To start, please fill out our registration form!
Note, if you are registering for a PRIVATE BAPTISM, you must submit the corresponding Registration Form and your child’s birth certificate , have godparents complete their respective Godparent Eligibility Forms AND have then send us their sacramental records; only then call to set a baptism date. Kindly, place your child’s name in the subject line of any email where documents required for the baptism are being sent to us.

Send the birth certificate
Next, please submit a copy of your child’s birth certificate. You may email the certificate, drop it off at the office, or mail it in!
To email the birth certificate, please submit a clear photo of the entire certificate and include "Birth Certificate/Child’s Name" in the subject line. Contact us here.

Select a Godparent
At Mary Immaculate, we understand that selecting Godparents is an important part of the Baptismal process and life after Baptism! Godparents play a vital role in the faith life of their Godchild. Due to its importance, there are a few requirements. Please look through the requirements, and if the person you have in mind is eligible, have them fill out the following form!
Must be a Christian (One Godparent must be a Catholic)
At least 16 years of age
If Single, not cohabiting
If married, marriage was celebrated in the Catholic Church
If Catholic, participates in Sunday Mass and regularly takes communion
Promises to give good Christian witness and pray for the child they are sponsoring
Must have received the Sacraments of First Eucharist and Confirmation and be prepared to present their certificates of completion of these sacraments.
Godparent Requirements

The Sacrament of Baptism requires that at least one godparent is a practicing Catholic. However, a second individual of the opposite gender can stand in as a Christian Witness during the Baptism.

We want your selected friend to be a godparent just as much as you do! We are more than happy to help prepare your desired godparent to meet the requirements, just be aware that it may delay your desired Baptismal date. Contact us about your situation and we will gladly work with you to help your desired godparent complete his/her sacraments and other requirements prior to the Baptism.

Yes! The godparent need only submit a copy of his/her completion certificate to prior to the Baptism.

No. Each person may have up to two godparents. One male. One female.

At Mary Immaculate, each parent and Godparent are required to attend a preparation class prior to the child’s Baptism.

Submit Fees
Your registration is complete once you have submitted all fees and donations. First, there is a $50 fee due at registration which applies to private and group baptisms alike. In addition, there is a $50 recommended offering per child the presiding priest or deacon for a group baptism. For a private Baptism, the recommended offering is a minimum of $200.
Payments must be done online before the Baptism date; however, not before your child’s baptism date has been confirmed. When paying online, please write “Baptism Registration- Name of child and baptism date” in the Notes section of the payment.
